This is the Model Health Questionnaire for Parent Breed Clubs recently distributed by the AKC. Its purpose is to give breed clubs a way to collect information about the state of health in their breed. Several breed clubs have approached the AKC and expressed interest in using this internet-based software referred to in the Mini-Bull Terrier Breed Column in the AKC Gazette (May '98). As a result of this interest, this survey now collects data for additional breeds.
Information supplied here will be captured and converted to a standard data format. There it may be analyzed on behalf of participating breed clubs and/or made available to the club for its own analysis.
The survey is divided into the following basic sections:
