General Instructions:
To use DogTests(tm), you must first Register with us (Subscribe). . We then mail you an ID (your "Dogtag") number which will be your personal "key" to Submitting and Viewing health test information.
1) fill in onscreen by clicking on fields and typing in your answers. Then2) you print the completed form and lastly3) send to us for processing at the address on the form.
Because we want to make DogTests as useful to you as possible, your comments and suggestions are welcome and will receive serious consideration.

a) Remember you must be using Acrobat Reader VERSION 4 or higher.
b) If you are having trouble completing the on-line form with your computer, within Acrobat Reader, save the file from the website onto your hard disk using "File/Save As...". (Remember where you put it and what it's called if you rename it. We suggest you don't change it). Then within Acrobat Reader, "File/Open" the form that you've saved (usually on c:). You should be able to complete the form now, by simply placing your cursor over the fields and typing in contents.
Trouble Printing?
tips:
a) reduce printer output resolution to 300 dpi
b) if you are using a color inkjet, switch to b&w (grayscale) mode and print in "Good" or "Normal" mode, not "Economy" - be sure your printer output is clear and aligned - "jaggies" will not scan properly.
c) change to "print as graphic image" or similar language in the dialogue box that comes up when you print
d) check http://www.Adobe.com for printing tips. Because Acrobat Reader is an industry standard, Adobe may have encountered problems with your specific printer and already have online recommendations for using it with Reader 4.0.
